4. What are other requirements for filing original proceedings?

The procedures differ depending on the type of petition. Rules 56 describes the procedures for filing petitions for writ of mandate, certiorari or prohibition. See rules 56.5 and 60 for special requirements for writs of habeas corpus. Specific requirements for petitions to review Workers’ Compensation Appeals Board cases are covered in rule 57, rule 59 deals with requirements for petitions to review Agricultural Labor Relations Board and Public Employment Relations Board cases, and rule 58 covers review of Public Utilities Commission cases.

All petitions for extraordinary writ, other than habeas corpus and writs from administrative proceedings, must be accompanied by a properly completed face sheet, the Appellate Court Writ Petition Information Sheet. This form is available from the clerk’s office. In addition, a copy of this form can be found in the Fifth Appellate District’s Local Rules.
