Agency or Foreign Adoptions
If this is an independent, agency, or foreign adoption, fill out these papers. You can get them from the court clerk or from this Web site.
In every adoption, an investigator writes a report. This report gives important information to the judge about the adopting parents and the child.
The investigator will come out to your home and ask you questions You may also be asked to fill out some papers and provide copies of your records. In some adoptions, the investigator may decide to talk to others, such as your neighbors or teachers at the child's school.
When the report is done, the investigator will file it with the court and send you a copy. Now you can ask the clerk for a date for your adoption hearing.
Go to court on the date of your hearing.
Bring:
- The child you want to adopt.
- Adoption Agreement (form ADOPT-210).
- Adoption Order (form ADOPT-215).
- Adoption Expenses (form ADOPT-230).
- A camera, if you want a photo of you and your child with the judge.
If you are adopting an Indian child, you will also need to fill out:
- Adoption of Indian Child (form ADOPT-220);
- Parent of Indian Child Agrees to End Parental Rights (form ADOPT-225); and
- the appropriate Indian Child Welfare Act (ICWA) forms for your case. Click here for a list of all the ICWA forms.
NOTE: All California courts use the same basic set of forms. But some courts have special local forms, too. To see if you will need any special, local forms, contact your court clerk or check your court's Web site. The forms may be posted on their site. If not, the site will list the address and phone number of your local courthouse.
