If the other parent filed an answer and you and the other parent have an agreement, fill out these forms to show the court that you have an agreement.

Also file:

  • A stamped envelope with your address; and
  • A stamped envelope with the other parent's address.

FL-235Advisement and Waiver of Rights Re: Establishment of Parental Relationship (Uniform Parentage)The father signs this form to show the court that he: (1) agrees that he is the parent of the child and (2) understands the consequences of being the legal parent. 
FL-230Declaration for Default or Uncontested Judgment (Uniform Parentage-Custody and Support)This form gives the judge the information needed to sign the judgment based on the parents' agreement and without the parents going to court.
FL-250Judgment (Uniform Parentage-Custody and Support)

The judgment says which orders the judge is making in your case.

[In a default case, the judgment must match the requests in your Petition to Establish Parental Relationship (form FL-200)]

Attach your agreement to the judgment. Make sure you both sign your agreement and have your signatures notarized.

FL-192Notice of Rights and Responsibilities -Health Care Costs and Reimbursement Procedures and Information Sheet on Changing a Child Support OrderThis form is for your information only. Read it and attach it to your court order. 
FL-191Child Support Case Registry FormThis form registers the case in a national registry to help with enforcement. You must file it with the court clerk at the same time you file the judgment. 
FL-195/OMB 0970-0154Order/Notice to Withhold Income for Child SupportThis form orders the paying parent's employer to withhold child support from that parent's paycheck and send it to the other parent.Instructions for form FL-195
FL-190Notice of Entry of Judgment (Family Law-Uniform Parentage-Custody and Support)Tells both parents that the judge has signed the judgment.Instructions for form FL-190
