What Do I Need My Surgeon or Doctor to Fill Out for the Court?

Your surgeon has to write an affidavit or declaration telling the court that you have undergone gender reassignment surgery. Your surgeon can use Form NC-210. If your surgeon can’t do it, you can ask your doctor to do it, but many courts prefer that a surgeon write the affidavit.

It is very important that your surgeon or doctor does this right. It can't be done by a nurse or someone who is not a licensed physician.
