What if I Already Changed My Name or if I Want to Keep My Name but Still Need to Change My Gender?

There are forms to change only your gender. To get a court order changing your gender ONLY, you have to:

  1. Fill out the Petition for Change of Gender and Issuance of New Birth Certificate (Form NC-300) and make 2 copies.
  2. Fill out Notice of Hearing on the Petition for Change of Gender and Issuance of New Birth Certificate for Change of Name and Gender (Form NC-320) and make two copies.
  3. Fill out the Civil Case Cover Sheet (Form CM-010)
  4. Have your doctor fill out an affidavit telling the court that you have undergone gender reassignment surgery. Your doctor can use Form NC-310 or write out his/her own declaration.
  5. File all the forms and copies in the court in the county where you live.
  6. Go to your court date. You will also need the Order for Change of Gender and Issuance of New Birth Certificate (Form NC-330) for the judge to sign.
  7. Once you get your Order for Change of Gender, get a certified copy from the court. You will need this to change your birth certificate and driver's license.

If you are filing for gender change only, you do not need to publish your request in the newspaper!
