Fee waiver forms are below.
Some courts also require a form called "Plaintiff's Statement to the Clerk." To get this form, you can:
- Go to the clerk's office of the court where you'll file your claim.
- Look for the form on your county's superior court Web site.
- Mail the court a letter asking for the form and enclose a self-addressed, stamped envelope. Look for your court's address at County-Specific Court Information in the Small Claims section of this Self-Help Center.
If you're under 18 years old, you need a guardian ad litem to sue for you in small claims court. Tell the court clerk you need an "Application and Order for Appointment of a Guardian ad Litem."

Request to Dismiss Your Case
In many counties, when you file your claim (Form SC-100), you also get a form called Declaration of Military Status/Request for Dismissal.
Use this form to say which defendants you don't want to sue anymore or to dismiss the entire case.
